Conclusioni

I nati LP, destinati probabilmente ad aumentare nel numero nei prossimi anni, costituiscono una popolazione che non va medicalizzata senza reali motivi, ma non devono sorprendere le eventuali problematiche, la cui insorgenza deve essere prevenuta con un ricovero “monitorato”. Il personale che assiste alla nascita di un LP deve rammentare che sia la condizione LP sia l’esposizione del neonato a patologie materne (con particolare riferimento all’ipertensione e alle emorragie antepartum) sono fattori indipendenti d sono fattori indipendenti di morbidità neonatale, e deve dunque intervenire tenendo conto di tutte le immaturità d’organo tipiche di questo piccolo paziente. In particolare le problematiche immediate da affrontare possono riguardare la termoregolazione, la respirazione e il metabolismo energetico. Il LP presenta, inoltre, una vulnerabilità neurologica, in parte identificabile nelle note lesioni cerebrali ma, probabilmente, in buona parte di non facile identificazione, in quanto i processi di maturazione cerebrale, soprattutto delle funzioni corticali superiori, sono complessi e poco indagabili con i mezzi diagnostici attualmente disponibili.
